[SARES] Missing Person Worksheet

All,
    Attached is a copy of the SARES - Missing Person Worksheet -.  This 
is the form that I will be using any time SARES is activated by DPS for 
a missing person search.  I will try and explain the various sections 
for your info when you need a copy.

SARES Activation # - I will supply

DPS Case # - not required

Name, Race, Sex, Age - self-explanatory

Endangered - this is a legal term - - any young juvenile is considered 
/endangered/, anyone with medical condition is /endangered/

Height, Weight, Head Covering, Hair Color, Hair Length / Style, 
Complexion, Glasses, Facial Hair - self-explanatory

LSW (Last Seen Wearing) - all below should be self-explanatory

Additional Info - This area would be used to provide any additional info 
to assist in recognizing the MP.
