[SARES] SARES Message Form

All,
    Here is the "official" Message Form that will be used by SARES.  
Please note that the title is "SARES Message Form".  This form was 
designed by the study group that meets at Santa Clara County EOC every 
Thursday evening and was put together by Jerry Haag the MTV EC.  Santa 
Clara County OES has approved the use of this form in their EOC and 
Cherel as approved this as our manner of communications from field to 
SNY EOC.

    Please print out a supply of these message forms for your go kit.  
You will notice that each block has a small number in the upper right 
corner - corosponding instructions are found on the back (2nd page).  We 
are currently planning on getting printed copies for distribution to the 
members as well as NCR copies for the radio room.

    Again we request you print out a supply for your go kit.
